Significant Financial Interest Review Form
- To be completed at the time of submission of a new proposal (and annually thereafter if awarded). The University will not submit a proposal until such form(s) has been submitted.
- An updated form shall be completed and filed within thirty (30) days at any time when an Investigator acquires or discovers a new reportable SFI not disclosed in the last disclosure.
- When a new investigator is brought on to an existing project, the investigator must complete a new form.
Supplemental Significant Financial Interest Disclosure Form
- If the potential for a significant financial interest is disclosed in the Significant Financial Interest Review Form, then a mandatory submission of the supplemental form is required. The supplemental form goes to the Financial Conflict of Interest Research Committee for confidential review when a potential for financial conflict of interest in research is determined to exist.
- Annual submission as required by the Financial Conflict of Interest in Research Committee.
- Circumstantial re-submission is required whenever there is a change in the nature of a potential financial relationship with regard to research.
