    Compliance Form 1

    In an attempt to better serve researchers, we (ACS) are requesting that Principal Investigators follow  the Standard Operating Procedure for ordering. Investigators should submit the form below for each order, whether or not there is a cost for the animals.

    1. For new projects, please notify the animal care managers in writing at least SIX WEEKS prior to submitting your order.  This is to assure the availability of space, equipment, and personnel.

      Please submit the form below at least TWO WEEKS prior to the start of your study. This is to assure the preparation of rooms, caging, and feed.

    2. For existing projects, please submit the form below at least ONE WEEK in advance.
